To
be eligible for Wrap Around Services:
-
The
child must have a social worker
or a probation officer or a 26.5
case carrying worker and
-
All
caregivers want the child to remain in or return to their care
and
-
All
family members must be willing to fully participate in the Wrap Around
program and
-
At
least 1 child in the home must be at risk of / or currently placed in
a level 10 or higher facility or
-
At
least 1 child in the home must be at risk of / or currently be hospitalized
due to mental illness
Strength Based
Wrap
Around seeks to build on each family’s strengths as they work toward self-sufficiency
and away from agency support. Wrap Around helps each family to build
their natural supports, so they have a strong, comprehensive support network
that can assist them as they work to meet each child’s individual needs.
Team
Approach
Each
family will identify individuals to be a part of the team that will
support them as they
learn to maintain their child’s placement, manage their own crises and
gain necessary resources on their own. Although Wrap Around utilizes
a team approach, the success of the program ultimately depends on each
family’s willingness to fully participate in the program.
Case Management
Wrap
Around provides case management to each family
involved in the program. The case manager helps families to identify
their areas of need with an initial assessment. Once the areas of need
are identified, the case manager and parents work together to access needed
services and build on their pre-existing strengths to achieve the desired
growth.
The
Wrap Around program is designed to help families maintain placement
for children who would otherwise be removed from their care.
Wrap Around
Focuses
on keeping children in their current placement, whether it be with their
biological parent(s), with a legal guardian or in a foster home. Wrap
Around also seeks to return children from group homes and mental
institutions back to their community in a less restrictive placement.
Wrap Around is provided in collaboration with Tehama County Department
of Social Services, Probation, Mental Health and other community agencies
to provide the most comprehensive services to families participating in
the program. |
